Some of them are definately photoshop, not tilt-shift.

Yeah - to do it without photoshop you must use a Tilt Shift lens. They are expensive!

Some of these are clearly photoshopped, but with good attention to detail it’s tough to tell the difference between a true tilt shift and photoshopped image.
